2020 Wellness Deadline Approaching - Save $$!

The 2020 Wellness Program is designed to support your health and well-being and provides the opportunity to pay the lowest health insurance premium. You will need to complete the Online Health Questionnaire and three activities by logging into the  Wellness Portal.   Activities need to be completed by Oct. 30, 2020. 

Complete the process by 10/30/2020 - pay 12.5% of the monthly premium cost. (Single $116.43 / EE+1 $232.85 / Family $355.35) per month

Start the process but do not complete by 10/30/2020 - pay 17% of the monthly premium cost. (Single +$41.91 / EE+1 +$83.83 / Family +$127.93) per month

Do nothing by 10/30/2020 - pay 20% of the monthly premium cost. (Single +$69.85 / EE+1 +139.71 / Family +$213.21) per month

Note: Both employee and spouse need to complete the questionnaire and outlined activities in order to pay the lowest health insurance premium (only if spouse is enrolled in the MATC health plan). 

Log into the Wellness Portal to complete the Online Health Questionnaire and other activities. 

New participants: Register and create an account.  

You will need your company code 0049 and your user ID which is your 7-digit Cosmo ID number ◆ Spouse: use employee’s 7-digit Cosmo ID number plus ‘SP’ (example: 1234567SP) 

Returning participants: Log in with your username and password from last year
